BayCare HomeCare is currently in search of our newest Home Health Aide Per Visit who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

Facility : BayCare Home Care

Location : Bradenton, FL

Status : Per Visit

Shift Hours : 8 : 00 AM - 5 : 00 PM

Weekend Work : Occasional

On Call : Occasional

Home visits in assigned service area : Manatee County, FL

The Home Health Aide Per Visit Responsibilities include :

Working under the guidance and supervision of the staff nurse in providing personal care to the patients.

Following the patients' plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional.

Maintaining an open communication process with the patient, representative (if any), caregivers, and family.

Demonstrating competency with assigned tasks.

Complies with infection prevention and control policies and procedures.

Reports changes in the patient condition; and honors patient rights.

Required Certifications and Licensures :

CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program

Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

BLS (Basic Life Support)

Education :

• Preferred : High School Diploma or equivalent
