Title: Clinical Pharmacist

Cornell Scott-Hill Health Center is looking to add a Per Diem Clinical Pharmacist to our team.
The Clinical Pharmacist is responsible for delivering comprehensive pharmacy services within a patient-centered, team-based FQHC environment. This includes chronic disease management, Medication Therapy Management (MTM), collaborative care with providers, population health initiatives, and patient/staff education. The Clinical Pharmacist participates in care team activities, supports medication safety and regulatory compliance, and assists with dispensing and daily pharmacy operations as needed.
Responsibilities include but are not limited to:
Reviews and interprets medication orders for safety, appropriateness, and adherence to clinical guidelines.
Provides direct patient care through MTM, chronic disease management (e.g., diabetes, hypertension, anticoagulation), and medication optimization services.
Identifies, resolves, and documents medication related problems in accordance with facility standards.
Designs and implements evidence based drug therapy monitoring plans.
Performs CLIA waived testing when appropriate and in compliance with regulatory standards.
Educates patients, caregivers, and community members on safe and effective medication use.
Performs all other duties as required.
Per Diem Requirements:
The position requires flexible availability from Monday through Friday 8:30AM to 5PM, and Saturdays from 8:3AM to 12:30PM, with shifts of up to 8 hours per day during the week, as needed. Responsibilities include providing coverage for colleagues' sick days and vacation time. To maintain a Per Diem status, the Clinical Pharmacist must work a minimum of 4 shifts per month, not exceeding 19 hours per week.
Requirements/Qualifications:
Degree from an accredited college of Pharmacy required. Residency preferred.
Licensed or eligible to practice pharmacy in the state of Connecticut.
Participates in American Society of Health System Pharmacists or other pertinent national pharmacy organization preferred.
Experience and familiarity with Joint Commission standards preferred.
Ability to communicate in a professional and effective manner with staff and patients in matters concerning drug therapy is required.
