Pharmacist

Arizona Home Care is a closed-door home infusion and homecare facility that was first established in 1995. Our service includes both acute and non-acute patient populations by bringing the hospital room to the patient's home. We work with all the major hospitals and providers in the valley including but not limited to: The Mayo, HonorHealth, Banner, Dignity, Valleywise, St. Joes, Hospice of the Valley, and Abrazo.

Our mission is to improve patient outcomes by being the best way home.

As a Pharmacist at Arizona Home Care, you will play a vital role in the care of our patients through:

1. Processing prescriptions and med refills
2. Creating accurate prescriptions
3. Communicate med change and cost changes to patients
4. Addressing side effects and abnormal labs with patients and provider
5. Supporting and leading a team of technicians
6. Prioritizing deliveries and backend drug verification based on delivery expectations

Qualifications:

• Licensed Pharmacist in the state of Arizona
• Friendly and confident on the phone
• Ability to spend long periods of time on your feet

Nice to Have:

• Experienced with our EMR: CareTend
• Experienced in home infusion
• Experienced in sterile compounding

Schedule:

• 10:30 AM to 7:00 PM Monday through Friday

Requirements:

• Bachelor of Science in Pharmacy with a current Arizona pharmacist license.
• Master of Science or Doctor in Pharmacy desirable.
• Continues completion of appropriate CEU's for licensure.
• Requires experience and a strong knowledge base in intravenous admixture services and intravenous therapy.
