Key Responsibilities

• Provide thoughtful, resident-centered care based on individualized plans, including medication administration, wound care, and vital sign monitoring during PM shifts.
• Assess and track changes in residents' conditions, collaborating with the oncoming night team for seamless handoffs and proactive updates to families, physicians, and interdisciplinary staff.
• Foster a calm, engaging evening environment through rapport-building, light activities, and emotional support, helping residents unwind while promoting dignity and independence.
