Part-time Post Acute Care Opening in Endicott, NY Binghamton 10m, Scranton PA 67mWe are seeking a part-time physician to provide patient care in a post-acute care setting in a skilled nursing facility. You will round 2 days a week with the flexibility to choose your rounding schedule. Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Collaborate with clinicians, facility leaders and others regarding care and services Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Provide oversight, guidance, and mentorship to our nurse practitioners Conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care

• Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues Qualifications Minimum of 1 year primary care experience Post-acute care experience a plus Active and unrestricted New York license or application in process Active and unrestricted DEA / prescriptive authority or application in process Compensation Compensation is fee-for-service. Expected compensation is estimated salary range of $96,900 to $116,280 annually with no cap on productivity income potential. Paid professional liability insurance with tail coverage
