Essential Functions Statement(s)
• Manage patient care and scheduling.
• Maintain accurate, detailed reports and records.
• Process, validate, and analysis of OASIS.
• Create 485 from OASIS assessment.
• Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.
• Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
• Monitor all aspects of patient care, including POC being carried out as written and goals met.
• Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs.
• Ensure the team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.
• Inform physician of patient's condition as required.
• Observe nurses and visit patients to ensure proper nursing care.
• Professionally manage patient, staff, and doctor issues as needed.
• Act as a liaison between the doctor and staff.
• Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.
• Recommend where appropriate drugs, medical devices or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures.
• Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing.
• Engage in training or research activities related to nursing.
• Perform other duties as assigned by management.

SKILLS & ABILITIES
Education : Registered Nursing Program
Experience : At least one year of Home Health experience required, two years related experience preferred.

- Valid Louisiana RN license without restrictions.

WORK ENVIRONMENT
Primary or Remote office and occasional patient home visits.
