Job Requirements & Qualifications:
- Develops activities providing the opportunity for residents to experience sensory input - touch, smell, taste, etc., group interaction and personal achievement.
- Evaluates each resident according to his/her background, interests, abilities, physical and mental limitations and needs. This shall serve as the base from which the individual activities program shall be developed. Provides accurate data as part of the MDS inter-disciplinary team.
- Documents/evaluates the individuals activity program using the appropriate assessment forms to be placed in the medical record, completing the activity care plan within seven working days of admission.
- Develops appropriate records, which indicate residence attendance and participation in the program with reference to resident response to the program.
- Develops a method to implement programs within a designated budget, allocated by the facility administrator.
- Attends and participates in staff meetings, department head meetings, designated committee meetings and resident care plan conferences. Also, shall attend mandatory staff meetings.
- Coordinates and Supervises special events, field trips and encourages residents to remain active in the community.
- Coordinates resident council meetings and ensures follow through on appropriate forms reviewed and signed by the facility administrator.
- Other duties as assigned.
- Two years of experience in a social or recreational program within the past five years, one of which was full-time in a resident activities program in a healthcare setting.
- Certified by the National Certification Council for Activity Professionals.
- In Lieu of the certification in item 2, may be an Occupational Therapist, Art Therapist, Music Therapist, Dance Therapist, Recreational Therapist or any Occupational Therapy Assistant.
- Sufficient verbal and written communication skills to perform the task required.
- Must have a valid driver’s license with clean driving record.
