Key Responsibilities:

• Work collaboratively with Physical Therapists to execute individualized treatment plans.
• Support and guide patients through exercises, functional tasks, and daily living activities.
• Interpret evaluations and liaise with physicians to optimize therapy approaches.
• Maintain precise documentation and comply with legal and professional standards.
• Conduct follow-up visits for home health patients to assess eligibility for continued services.
• Provide maintenance therapy and assist patients in reaching their rehabilitation objectives.
• Regular travel to patient residences is required, necessitating a valid driver’s license.

Qualifications:

· Home Health experience preferred

· Valid California Physical Therapist Assistant license required

· Current CPR certification required

· Current California driver’s license, good driving record, and reliable transportation
