Verovian Nursing Recruitment Agency is seeking a dedicated and experienced RN Care Manager to join the Care Management Team at Lancaster, CA. This full-time position requires availability for 8-hour variable shifts, including 8:00 AM - 4:30 PM or 4:30 PM - 1:00 AM with weekend rotations.

As an RN Care Manager, you will play a critical role in care coordination, utilization management, and transitional care planning to support patients throughout their healthcare journey. You will collaborate with patients, caregivers, physicians, social workers, and other healthcare professionals to ensure seamless and efficient patient care while optimizing healthcare resources.

• Coordinate and facilitate communication between patients, caregivers, and interdisciplinary healthcare teams.
• Support care transitions and discharge planning, ensuring continuity of care across acute, ambulatory, and post-acute settings.
• Assess and address social determinants of health, providing resources and advocacy for patients.
• Utilize case management strategies to enhance patient outcomes, reduce readmissions, and improve hospital efficiency.
• Ensure compliance with utilization review guidelines, payer policies, and hospital regulations.
• Implement evidence-based practices in care management, focusing on quality patient-centered care.
• Participate in weekend rotations and support the hospital's commitment to clinical excellence.

Candidate requirements

Required:

• Bachelor's Degree in Nursing (BSN) OR an equivalent education/experience combination.
  • RNs without a BSN must obtain one within 3 years of hire.
• California Registered Nurse (RN) License - Upon Hire.
• California Fire and Life Safety Card - Within 30 Days of Hire.
• 2 years of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.).
• 2 years of experience in Case Management (Care Coordination or Utilization Management) OR successful completion of the Transitions in Practice (TIP) program for Care Managers.

Preferred:

• Master's Degree in Nursing or a Healthcare-related field.
• Certification in Case Management (ACM or CCM) - Upon Hire.
