We're looking for a strategic and compassionate professional to lead our Case Management Operations and help shape services that truly make a difference.
In this role, you will lead two dynamic teams supporting individuals with developmental disabilities who participate in Day Support, Respite, and Summer Camp programs, as well as employees with disabilities working in integrated employment settings. If you are a collaborative leader who values mission, accountability, and innovation, this is an opportunity to make a meaningful impact.
Key Responsibilities:
As the Manager of Case Management Operations, you will provide leadership and direction to teams of Case Managers and Intake Coordinators who coordinate services and supports across multiple programs. You will oversee assessments, individualized planning, and coordinated care to ensure every participant receives services tailored to their goals and strengths.
You will also play a key role in shaping program quality by:
Leading and developing high-performing case management and intake teams
Ensuring services remain person-centered, responsive, and outcome-driven
Partnering with internal teams to strengthen collaboration across programs
Building relationships with community organizations and referral partners
Educating stakeholders on best practices for supporting individuals with disabilities
Monitoring caseloads, program performance, and service quality
Ensuring compliance with Medicaid Waiver, AbilityOne, and other regulatory standards
