Care Coordinator

As a Care Coordinator, you'll play a vital role in making a positive impact on the lives of children and adults with behavioral or mental health challenges. You'll work closely with clients and their families, meeting them in their homes to understand their strengths and assess their emotional, behavioral, and physical needs. By connecting them to essential community resources, you'll create tailored treatment plans designed for real progress. You'll engage clients and families in their journey through regular check-ins, celebrating milestones and fostering success. Our Care Coordinators work closely with a team of educational, vocational, medical, and behavioral health providers to ensure wraparound support that empowers clients to thrive long-term.

What we’re looking for :

Bachelor’s degree in any field

MHRT / C Required
