About the Position

LocumJobsOnline is working with AAS Locum Tenens to find a qualified Family Practice NP in Mishawaka, Indiana, 46546!

We are seeking a compassionate and experienced Nurse Practitioner to join our team in Mishawaka, Indiana. This full-time role works closely with clinical staff and reports to the Site Medical Director and Site Operations Director, providing high-quality, patient-centered outpatient care within a Patient-Centered Medical Home (PCMH) model.

Responsibilities:

• Provide comprehensive outpatient primary care and manage patient panels to ensure continuity of care
• Develop and implement effective patient care plans, including referrals as appropriate
• Perform periodic health assessments and provide patient education
• Maintain timely, accurate, and compliant clinical documentation
• Complete a minimum of 2,946 outpatient visits annually
• Participate in quality assurance activities, clinical meetings, and performance audits
• Support quality improvement initiatives and clinical protocols
• Participate in community outreach, education, and student precepting as assigned

Requirements:

• Master's Degree in Nursing from an accredited Nurse Practitioner program
• Active Indiana RN license and Nurse Practitioner certification
• Prescriptive authority with active DEA registration
• Current CPR certification
• 1-3 years of Nurse Practitioner experience or significant RN experience in ER or ICU
