Minimum Requirements :

Licensed to practice medicine plus four years of appropriate experience. Completion of a residency program may substitute for the required experience.

Preferred Qualifications :

• Certification / Licensure in the state of Florida and Federal DEA Certification.
• Medical Degree M.D., D.O., or foreign equivalent.
• Board certified in the appropriate subspecialty.
• Completion of an accredited residency program.
