Illinois Licensed Case Manager RN

A company is looking for a Case Manager Registered Nurse, Assessor Team.

Key Responsibilities

Develop proactive care plans to enhance member wellness and outcomes

Conduct evaluations of member needs using clinical tools and data

Collaborate with stakeholders to overcome barriers and meet healthcare goals

Required Qualifications

Minimum 3-5 years of clinical practical experience

Minimum 2-3 years of experience in Care Management or home health care coordination

Must possess an active and unrestricted Registered Nurse license in Illinois

Associates degree required; Bachelor's degree preferred

Confidence in working independently and using virtual collaboration tools
