Requirements:

• MDor DO degree from an accredited medical school.
• Completedresidency in Family or Internal Medicine accredited by the AccreditationCouncil for Graduate Medical Education

• Activeand unrestricted medical license to practice in Arizona.
• CurrentDEA registration for prescribing controlled substances.
• Priorexperience in a family medicine setting preferred.
