About the Position

NPORT Healthcare Staffing is seeking a dedicated and compassionate Physical Therapist (PT) to provide high-quality therapy services within a Skilled Nursing Facility (SNF) Setting. The role may also extend care for patients in Home Care (CHHA) to ensure continuity of rehabilitation, offering a rewarding opportunity to support individuals across different levels of care in New Windsor, NY.

Key Responsibilities

• Evaluate patients to determine functional status and individualized care needs.
• Develop and implement effective treatment plans for residents in an SNF and / or home care setting.
• Provide therapeutic exercises, gait training, balance activities, and other PT interventions.
• Collaborate with an interdisciplinary team including nursing, OT, speech therapy, and social services.
• Maintain accurate and timely documentation in accordance with professional and regulatory standards.
• Educate patients and families on therapy goals, safety practices, and home exercise programs.
• Assist in discharge planning and care coordination as needed.
