Position : Admissions Support Coordinator

Location : Monmouth Medical Center Monmouth County, NJ

Shift : 8 : 30am-5 : 00pm

Purpose : The person in this position will provide support to the Grace Hospital Liaisons.

Key Responsibilities :

• Track referrals working with the team and Intake to ensure that all information is given to Intake so the referral can be processed.
• Meet with families to secure consents, explain the hospice benefit and ensure expectations are set.
• Work with hospital Case Management and Social Worker's along with the onsite Grace team
• Order DME and equipment at the direction of the Grace RN
• Other duties as assigned within the persons scope of practice by the Grace Hospital Liaisons
• Ensure that Grace Healthcare Services' Best Practices are incorporated into all job activities, and responsibilities.
