Home Health Occupational Therapist

POSITION SUMMARY

We seek a dedicated and compassionate Occupational Therapist to join our Home Health team. The ideal candidate will provide personalized, in-home therapy services to patients recovering from illness, injury, or surgery. This role focuses on helping individuals regain independence in their daily activities and improve their overall quality of life in the comfort of their homes.

QUALIFICATIONS

• Accredited occupational therapy degree.
• Current registration or license as required by the state(s) of practice.
• CPR certification.
• Strong verbal and written communication skills.
• Licensed driver with an insured automobile and a safe driving record.

ESSENTIAL FUNCTIONS

• Perform assessments, diagnostic tests, treatments, and evaluations under a medically approved Plan of Care.
• Develop treatment programs and establish goals for improved function.
• Document findings, plans, interventions, and client progress.
• Communicate plans and changes to the physician and Agency staff.
• Guide and instruct clients in therapeutic self-care and activities.
• Advise and teach treatment procedures to clients, family members, and Agency staff.
• Design and order adaptive/assistive devices.
• Comply with Agency policies and professional standards.
• Attend care conferences and participate in care coordination.

SUPERVISION OF OCCUPATIONAL THERAPY ASSISTANTS

• Instruct and supervise Occupational Therapy Assistants.
• Available by telephone to assist Occupational Therapy Assistants.
• Supervise on-site every four to six visits, minimally once a month.
• Complete on-site functional assessments and review the Plan of Care.
