Key Responsibilities:

• Deliver comprehensive medical care for diverse patients with acute and chronic illnesses, maintaining an independent oversight of patient outcomes.
• Act as a health coach, guiding patients in setting and achieving their health goals while fostering trust and partnership.
• Work collaboratively with care teams, including advanced practitioners, to ensure patient needs are met effectively.
• Engage actively with patients during hospitalizations and actively participate in care transitions.
• Monitor and manage care coordination for patients unable to attend office visits.
• Play a leadership role in enhancing center culture and supporting team recruitment efforts.

What We Are Looking For:

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
• Active medical license and board certification (or eligibility) required.
• Expertise in evidence-based medicine and a drive for continual professional development.
• A strong focus on quality, service orientation, and compassionate patient care.
• Excellent communication skills, both verbal and written, with proficiency in modern medical software and technologies.
