Job Duties

As a Community Health Nurse, you will play a vital role in promoting the health and wellness of individuals within Manhattan community. Your responsibilities will include:

Conducting health assessments and screenings for community members.

Developing and implementing personalized care plans for patients with varying health needs.

Providing education on preventive measures and health promotion strategies.

Collaborating with healthcare professionals, community organizations, and local resources to ensure holistic care.

Administering medications, treatments, and vaccinations as needed.

Maintaining accurate and up-to-date medical records.

Participating in health education events, workshops, and community outreach programs

Experience & Skills Required

Proven experience in community or public health nursing.

Strong communication and interpersonal skills.

Ability to work collaboratively in a multidisciplinary team.
