A company is looking for a Manager of Practice Transformation to oversee the Practice Transformation Specialists and ensure effective operations in compliance with policies and contractual requirements.
Key Responsibilities
Manage and develop staff to ensure alignment with organizational goals and effective team performance
Provide training and coaching to network providers, focusing on continuous quality improvement and data-informed care management
Collaborate with internal and external stakeholders to enhance relationships and resolve issues effectively
Required Qualifications
Master's degree in Social Work, Counseling, Healthcare Administration, Public Health, or Business Administration
Three years of post-degree experience in healthcare quality, population health, or care management
Previous supervision or management experience preferred
Understanding of Health Information Technology (HIT) and its role in healthcare outcomes
Moderate analytic knowledge and project management skills
