Meeting the needs of the whole person is our continued commitment.What You'll Do:The RN Clinic Manager, is responsible for overseeing the delivery of high-quality, participant-centered clinical care across all PACE settings. This position ensures that clinical operations align with the PACE model of care, delivering high-quality, participant-centered services that promote independence, health, and well-being for PACE participants. The Clinical Supervisor, RN, collaborates closely with interdisciplinary team (IDT) members to integrate clinical services effectively. Key responsibilities include oversight of clinic operations and staff, clinical compliance, quality improvement initiatives, and clinical staff development. Position ResponsibilitiesProvide strategic direction and clinical oversight for day-to-day operations within the PACE clinic.Manage clinical scheduling, staffing levels, and resource allocation to ensure adequate coverage and continuity of care.Supervise, mentor, and evaluate nursing and clinical support staff, fostering professional growth and a culture of accountability.Support onboarding and training of new clinical team members to ensure competency in PACE care standards.Oversee participant care coordination, ensuring timely assessments, interventions, and follow-up according to individualized care plans.Monitor clinical workflows to optimize efficiency, participant experience, and care quality.Ensure compliance with all federal, state, and PACE regulatory requirements, policies, and quality standards.Serves as a nursing subject matter expert and communicates best practices for a frail elderly population to the IDT, participants, and caregivers.Provides case management and care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care planspletes and maintains documentation in the PACE EHR in accordance with the federal PACE regulation and organizational policy. What You'll Bring:Experience & Education(CMS required) Graduate of an accredited School of Nursing.(CMS required) Current RN license in good standing to practice in StateName.(CMS required) One year of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years.(Preferred) Minimum 3 years RN clinical experience.(Preferred) Experience working in Electronic Health Records (EHRs).(CMS required) Meet a standardized set of competencies for the specific position description established by PACE before working independently.(CMS required) Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.(CMS required) Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act.(CMS required) Have not been excluded from participating in Medicare and Medicaid programs.(CMS required) Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.(Preferred) BLS certified.
