A Physical Therapist (PT) specializing in Home Health services is needed in Springfield, Illinois. This position involves providing patient care in a home health setting, focusing on rehabilitation and therapy services. The role is available starting August 10, 2026, offering a supportive environment to deliver quality care in a community-based setting.
Job Details:
- Location: Springfield, IL
- Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Valid Physical Therapist (PT) license, applicable state licensure, graduate degree in Physical Therapy, minimum of 2 years of experience in home health, and current CPR certification
- Preferred Qualifications: Experience with electronic medical records, familiarity with Medicare/Medicaid regulations, and additional advanced certifications in rehabilitation or therapy
- Float: Float rotation may be required
- Additional Information: The position involves conducting patient assessments, developing treatment plans, and coordinating care with other healthcare professionals. Candidates must be committed to patient-centered care and adhere to all clinical guidelines and protocols.
