Job description

SummaryCatholic Charities of Acadiana is seeking a compassionate, organized, and mission-driven Case Manager II to provide comprehensive support to individuals experiencing moderate levels of crisis, homelessness, or instability. This mid-level role is ideal for someone with case management experience who is ready to take on more complex service needs and help clients build greater stability and resilience. As a Case Manager II, you will provide trauma-informed, strengths-based case management primarily to Tier 2 clients, helping them identify goals, address barriers, and navigate systems such as housing, healthcare, behavioral health, employment, and public benefits. You will develop individualized service plans, coordinate care with internal programs and community partners, and provide supportive, non-clinical counseling to help clients work toward long-term stability. This role requires strong problem-solving, communication, and organizational skills, as well as the ability to recognize when a client's needs require additional support or escalation. This is an excellent opportunity for an experienced social services professional who wants to make a meaningful impact while helping individuals move from crisis toward greater independence and stability in the Acadiana community.Key ResponsibilitiesProvide comprehensive case management services primarily to Tier 2 clients experiencing moderate service needs, homelessness, or instabilityMeet with clients to conduct strengths-based assessments, identify barriers, establish goals, and determine appropriate service pathwaysDevelop individualized service plans with measurable objectives and support clients in working toward identified goalsProvide supportive, non-clinical counseling, including motivational interviewing, emotional support, and trauma-informed de-escalationCoordinate access to housing, behavioral health care, medical care, employment services, public benefits, and other essential resourcesHelp clients navigate complex systems, including healthcare, behavioral health services, housing authorities, and community-based programsCoordinate care with internal departments, external providers, and community partners to ensure consistent and client-centered service deliveryConduct field-based visits in shelters, community settings, or client residences as neededAssist clients experiencing heightened stress or instability and participate in safety planning and coordinated crisis responses when appropriateRecognize when client needs require escalation to a Case Manager III or Clinical Case ManagerMaintain accurate, timely, and complete electronic documentation, including service plans, progress notes, outcomes, and referralsTrack client progress and service outcomes to support program reporting and continuous improvementUphold confidentiality, ethical standards, professional boundaries, and trauma-informed practicesParticipate in team meetings, case consultations, supervision, trainings, and professional developmentRepresent the mission and values of Catholic Charities of Acadiana through hospitality, professionalism, compassion, and respectPerform other duties as assigned by the supervisorJob QualificationsBachelor's Degree in Social Work, Psychology, Human Services, or a related field preferred; Master's Degree preferredMinimum 1 year of case management or social services experience requiredExperience working with homelessness, behavioral health, crisis situations, or other complex social service needs strongly preferredDemonstrated ability to build rapport, communicate effectively, and maintain professionalism with individuals from diverse backgroundsUnderstanding of trauma-informed care and harm reduction principles preferredStrong organizational, time-management, problem-solving, and documentation skillsAbility to manage multiple cases, priorities, and service coordination needs effectivelyProficiency with electronic documentation systems and Microsoft Office SuiteKnowledge of local community resources, public assistance programs, housing systems, and social service networksAbility to work collaboratively with clients, coworkers, service providers, and community partnersJob SpecificationsHeavy: Exerting 50 to 100 pounds of force occasionally, or 25 to 50 pounds of force frequently, or 10 to 20 pounds of force constantly to move objects.
