Credentialing & License Verification As part of our screening process, our Credentialing Department will verify applicable professional licenses, DEA information, and NPI/taxonomy information.

Candidates should be prepared to provide the information needed to complete the credentialing process.

Licenses and credentials must be active, in good standing, and within the provider's scope of practice.

What You'll Do Provide patient-centered care focused on hormone replacement, weight management, and targeted wellness Conduct patient assessments and develop individualized treatment plans Provide follow-up care and monitor patient progress Educate patients on treatment plans, wellness, and lifestyle goals Build genuine relationships and establish trust with patients Explain medical information in a clear, easy-to-understand way Collaborate with the Center Director, Medical Director, and clinic team Maintain accurate patient documentation and follow established clinical protocols Help create a positive, welcoming, high-energy patient experience We're Looking for the Right Personality Clinical skills matter.

• Friendly, approachable, and energetic A strong communicator Comfortable building relationships with patients Compassionate and patient-focused Confident making clinical decisions A team player who enjoys collaborating with others Positive and adaptable when things get busy Comfortable in a fast-paced environment Tech-savvy and comfortable learning new systems Someone who genuinely enjoys helping people Required Qualifications Active Indiana NP or PA license in good standing Current DEA license or ability to obtain Strong communication and clinical decision-making skills Clinic Hours Monday: 8:00 AM–5:00 PM Tuesday: 8:00 AM–7:00 PM Wednesday: 8:00 AM–1:00 PM Thursday: 7:00 AM–5:00 PM Friday: 8:00 AM–5:00 PM Saturday: 8:00 AM–12:00 PM 2 weeks of paid training (10 consecutive business days) are provided to help you learn our clinical protocols, systems, and workflows.
