About the Role:
As a Post-Acute Care Physician, you will provide care for a panel of patients in skilled nursing and rehabilitation facilities.
In this role, you’ll partner with facility staff and Trumen affiliated clinicians to play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.
