Legacy Lifecare VNA West | Travel Required Longmeadow/ Springfield, MA and surrounding areas We are seeking a compassionate and skilled RN Case Manager to provide high‑quality primary nursing care to patients in their homes and Assisted Living communities.

What You'll Do:

• Complete initial and ongoing patient assessments and develop individualized plans of care Provide skilled nursing services, including preventative and rehabilitative care Monitor patient progress and respond promptly to changes in condition Educate and support patients and families Coordinate care with physicians, caregivers, and home health team members Maintain accurate and timely clinical documentation Supervise and support home health aides as needed What You'll Need: Active unencumbered MA Registered Nurse (RN) license Graduate of an accredited school of nursing Minimum 1 year of RN nursing experience CPR certification IV therapy experience a plus Benefits Available to You: Comprehensive benefits package, including Health, Dental, Vision, company-funded Life Insurance, and optional STD, LTD, Accident, and Critical Illness coverage.
