MDS Coordinator (LPN)
Full-Time | Day Shift
We are seeking an experienced and detail-oriented MDS Coordinator (LPN) to manage and coordinate the resident assessment process in accordance with federal and state regulations. This role is critical to ensuring accurate clinical documentation, regulatory compliance, and appropriate reimbursement.
This position is ideal for an LPN with strong organizational skills and a solid understanding of the RAI/MDS process in a skilled nursing environment.
Responsibilities
• Coordinate and complete the RAI/MDS process in compliance with Medicare, Medicaid, and OBRA requirements
• Maintain accurate assessment schedules for all payer sources
• Ensure timely completion and transmission of MDS assessments
• Monitor Quality Measures (QMs) and Quality Indicators (QIs) and initiate corrective actions as needed
• Chair or participate in Daily Case Management, UR, and Triple Check meetings
• Collaborate with the interdisciplinary team to ensure assessments reflect resident status and services provided
• Validate ARDs and coordinate with therapy services for PPS pathways
• Ensure documentation supports reimbursement and regulatory compliance
• Prepare for audits, surveys, and reviews related to MDS and reimbursement
• Provide education and guidance to staff on the RAI process
Qualifications
• Current, active LPN license in the state
• Minimum two (2) years of MDS Coordinator experience in a skilled nursing facility
• Strong knowledge of RAI/MDS regulations and reimbursement processes
• Excellent attention to detail and organizational skills
• Ability to work independently and meet strict deadlines
• Strong communication and interdisciplinary collaboration skills
