Case Manager, CHOICE Youth Development Services


Position Summary

Under the supervision of the Youth Development Program Manager, utilize a team approach to provide comprehensive case management, mentorship and educational services to youth on probation and their families participating in the CHOICE program. Services include, but are not limited to, in-home support, parent training, crisis intervention, mentorship, case management, conducting placement assessments, transporting participating youth, and community education and development. The Case Manager is expected to function as part of a multi-disciplinary team while making sound, independent decisions for the benefit of the clients and their families. The Case Manager works in a culturally sensitive manner and abides by the legal and ethical standards of the profession. Due to the nature of the workflow and client needs, the CHOICE Case Manager must be available to work schedules that include days, evenings, and weekends to best meet needs of the population served.


Specific Activities
Duties include but are not limited to:
· Serves as a positive role model following CHOICE guidelines for professional dress and behavior
· Conducts client intakes and comprehensive assessment interviews with youth and family/caregiver
· Assists in assessing clients and their families on an ongoing basis
· Develops and maintains supportive relationships with parents or guardians emphasizing client accountability
· Provides intensive tracking services
· Provides client advocacy in the areas of health, education, vocation and family intervention
· Creates and leads recreational activities for individual as well as groups of clients
· Uses behavior modification techniques, including positive reinforcements and contracts with parental involvement
· Administers appropriate procedures when client safety is threatened
· Develops linkages to community resources
· Strengthens working relationships with significant resources for the client network (e.g. Probation Dept., court personnel, Dept. of Social Services, schools, police, clinics, work sites, etc.)
· Participates in on-call system to ensure client coverage
· Participates in a team approach, sharing responsibility for client services
· Transports clients in a safe and secure manner in matters related to service delivery (e.g. fieldtrips, outings, and workshops)
· Participates and assists in implementing program components
· Develops and implements strength-based individual and family case plans
· Provides family focused psycho-education utilizing evidenced based strategies and a trauma informed approach
· Provides crisis intervention and stabilization
· Case conference in coordination with other mental health professionals including psychiatrists, psychologists, social workers and other case managers
· Participates in weekly multi-disciplinary team meetings to develop appropriate interventions for youth and families, and serves as a consultant for the team in their various areas of expertise
· Provides in-home family support and case management (e.g. nutrition, budgeting, safety, home health and support systems), including developing family interventions
· Advocates, mentor and role-model skills for families such as discipline, communication and general life skills
· Conducts and supports specific groups as necessary, e.g. parenting class, decision-making, substance abuse, anger management and life skills
· Creates and leads prosocial activities for clients and families as necessary
· Provides educational counseling to encourage school attendance and provide referrals to tutoring
· Assists in career guidance, job skills and job placement for youth
· Provides assistance with community resources through advocacy, information and direct connections
· Receives referrals from and provide progress reports to Juvenile Court, Juvenile Probation and Law Enforcement entities as specified by the CHOICE Program
· Participates in component meetings and trainings
· Maintains necessary client records, statistics, and intensive tracking procedures and submit timely monthly reports including entering all data into the Efforts to Outcomes (ETO) database
· Participates in case supervision and program evaluation
· Participates in quality assurance review
· Participates in agency outreach events
· Provides administrative duties and meets all contract requirements
· Participates in contract related and continuing education trainings
· Performs other related duties as assigned


Required Education and Experience
B.A. or B.S. degree, in social work, psychology or related field, experience may be substituted. Bilingual (English/Spanish) preferred. Minimum 1-year experience in child, youth and family services field. Experience should preferably have been in a community based or neighborhood service organization. Experience in the areas of individual, group, and family counseling, outreach, community education and development, advocacy and case management. Ability to work with a diverse cultural population.

Must demonstrate knowledge of trauma Informed, client driven, and culturally competent care.

Other Requirements

Reliable transportation. Must have a California driver license and good driving record. Willing to use own, properly insured, reliable vehicle for the job (mileage reimbursed). Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee may be exposed in an extremely stressful environment where there are considerable mental and emotional demands. Travel is required to client homes. Employee may be exposed to weather conditions prevalent at the time. The employee will be required to operate office equipment requiring repetitive hand movement and fine coordination including use of a computer.
