Were looking for aLicensed Clinical Social Worker (LCSW)who thrives in a collaborative, multi-specialty environment and is passionate aboutbreaking down barriers to care. In this role youll be in the room where it happens, making real-time, life-changing interventions for patients when they need them most. What Youll Do Providebrief, evidence-based interventions(often 68 sessions) focused on improving functioning and supporting behavioral change.Conductbehavioral health assessmentsand collaborate with primary care teams on shared treatment plans.Offer immediate support during medical visits throughwarm hand-offsand on-demand consultations.Address a range of issues, including depression, anxiety, trauma, chronic disease management, stress, and substance use.Makereferrals to specialty mental health serviceswhen higher levels of care are needed.Maintain timely, clear documentation and contribute to a strong culture of team-based care. Who You Are You are aCalifornia Licensed Clinical Social Worker (LCSW)in good standing.You have experience providingbrief, solution-focused therapyin high-volume or primary care settings.You understand the value oftrauma-informed, culturally responsive care, and can build trust quickly.Youre comfortable thinking on your feet and making clinical decisions in real time.Youre a collaborator, educator, and advocate for health equity. Preferred Qualifications Bilingualin Spanish, Punjabi, or Portuguese (a major plus in our diverse community).Prior experience working inFQHCs, school-based health, or integrated care settings.Familiarity withEHR systemsand clinical documentation workflows.
