Job Title: RN Home Health Profession: Registered Nurse Specialty: Post Acute Duration: 13 Weeks Shift: Day Hours per Shift: 8:30am - 5pm Experience: Minimum 1 year Medical/Surgical Nursing required; Home Health and ICU experience preferred License: RN - Registered Nurse (single state) or RN - Multi-state Compact Certifications: BLS - Basic Life Support Must-Have: Strong critical care nursing skills; computer/data entry experience; ability to communicate with various members of the healthcare team; knowledge of Microsoft Excel and Word; basic computer operational knowledge. Description: The RN Home Health is responsible for providing initial, resumption, and recertification evaluations of patients in their homes. This role includes performing routine home health visits that address the physical, emotional, developmental, psychosocial, and educational needs of patients. The RN Home Health coordinates the initial disciplines to begin care and plans the care throughout the patient episode. Collaboration with the physician, patients, family members, and team managers is key to developing the expected patient visits and scheduling them appropriately. The RN Home Health communicates with team members, including RNs, LPNs, therapists, social workers, home health aides, and the physician to ensure the patient's plan of care is comprehensive and effectively developed. Core responsibilities involve assessing, coordinating, and managing the total needs of patients. This includes following physician orders and using the nursing process to develop a patient's plan of care. The RN Home Health performs both routine and complex nursing procedures to meet patient needs. Collaboration with the physician, patient, family, and team manager is essential to developing the plan of care for each patient. The RN Home Health develops care plans for home health aides. Effective communication with physicians is necessary to report any changes that may affect patient recovery. Patient and caregiver education is a critical component of this role to develop and encourage self-care. Utilizing both verbal and written tools is essential for facilitating learning and promoting independence during recovery. It’s important to report any concerns regarding patient or staff safety to the appropriate authorities. The RN Home Health communicates patient status changes and lab results to the physician as necessary. Participation in care team conferences is required. Checking voice mail and staff messaging daily is part of the responsibilities. Demonstrating knowledge of regulatory guidelines is crucial, and accurate completion of documentation is required. The RN Home Health participates in the orientation and education of new employees as appropriate. Continuing education and in-service training are important for maintaining current clinical knowledge. Participation in performance improvement activities is encouraged. Identifying methods for improving departmental performance and care is part of the job. Meeting established productivity standards is expected. Weekend and call rotations may be required. Other duties may be assigned as necessary by leadership.
