Position Summary

As a member of the leadership team, the Program Manager empowers and leads a team of Case Managers and Activity Coordinators in providing person centered plans and disability services for the people served at the Rise Coon Rapids Program.


Responsibilities

Program Management

  • The Program Manager supports a team of case managers in communication with families, county case managers, and assist with problem solving efforts to improve service plans.
  • Developing and supporting the innovation of robust day services in alignment with wavier re-imagined.
  • Ensure compliance and quality of programs and services while maintaining working knowledge of 245D licensing, regulatory, and CARF quality standards.
  • Assist in responding to emergencies when needed and ensure documentation and follow up regarding incidents are completed.
  • Oversees and implements new program goals and ensures projects are completed in a timely manner.
  • Ensure efficient, timely and accurate data management, and documentation in our case management software system.
  • Build and maintains effective working relationships between all team members, persons served, and other program stakeholders.
  • Serve as a liaison with other appropriate agencies and organizations to promote the interests of Rise.

Leadership

  • Build and maintain a positive, effective work environment where people want to do their best.
  • Manage and grow team performance through direction, feedback, training, and professional development.
  • Lead others in focusing on providing excellent service for people served, their families, and all other people who support them.
  • The Program Manager monitors program outcomes, revenue, and expenses of program in partnership the Program Director.
Requirements:
  • 3 years of supervisory level experience in a program providing disability services is required.
  • A degree in a human service-related field.
  • Previous experience practicing person-centered practices with people with disabilities or other barriers.
  • Must meet the qualifications required of MN 245D statute's Designated Coordinator and Designated Manager roles.
  • Strong self-management, thinking and interpersonal skills.
  • Proficient with Microsoft Office software (Outlook, Word, etc.) and interest to learn and become proficient in case management software system (Care Director).
  • Applicants must comply with and pass Department of Human Services background check (Rule 11).
  • Applicants must have a valid driver license.
  • Must be able to pass a Motor Vehicle Record (MVR) background check.

Wage: $27.29 - $30.60 per hour depending on qualifications
