· Hospice Honors Elite Award Recipient:

o Hospice of NGMC receives this special recognition by scoring above the national and local average on all quality measures. This prestigious annual review recognizes hospices that continually provide the highest level of quality care as measured from the caregiver’s point of view.

Practice Overview:

· Dedicated hospice and palliative care teams serving patients for 40 years.

· Inpatient Hospice Unit at main campus- 7 bed unit

· Free standing 16 bed hospice currently under construction just minutes from main campus

· Provide support to nursing and interdisciplinary staff by providing oversight for home hospice and in-home palliative care

· Full-time and part-time schedule available

· Service Line supported by dedicated:

o Physicians and Advanced Practitioners

o Licensed Clinical Social Workers

o Chaplains

Working together with Nurse Practitioners like you:

· Board Certified FNP or AGACNP

· Active Georgia license or ability to obtain unrestricted license

· Passion for quality and patient care

· Experience in Palliative or Hospice Care preferred

· Functions well in a team environment, contributing to and supporting organizational goals and initiatives
