Contract Duration: 13 Weeks (Temp to Hire)
Orientation: 40 Hours (Non-Billable)
"Navitas Healthcare, LLC" is seeking a dedicated LMSW or RN Case Manager to coordinate patient care, facilitate discharge planning, and ensure appropriate utilization of healthcare services in an acute care setting. This role partners closely with interdisciplinary teams to improve patient outcomes, reduce length of stay, and ensure seamless transitions of care.
