Position Overview Inabia is seeking a Physical Therapist to join its rehabilitation team providing therapy services to residents in a skilled nursing and post-acute care environment.
The therapist will play a key role in improving patient mobility, strength, balance, and overall quality of life.
Both experienced therapists and motivated new graduates are encouraged to apply.
Key Responsibilities Conduct detailed physical therapy assessments to identify mobility challenges, pain levels, and functional limitations.
Develop individualized rehabilitation treatment plans designed to improve mobility, strength, and independence.
Provide therapeutic interventions including exercise therapy, gait training, balance rehabilitation, neuromuscular re-education, and functional mobility training .
Assist patients recovering from surgery, stroke, injury, or chronic health conditions .
Educate patients and caregivers on home exercise programs, mobility safety, and assistive device use .
Track patient progress and adjust treatment plans based on clinical outcomes.
Maintain thorough documentation in accordance with regulatory and facility requirements.
Collaborate with the interdisciplinary team including physicians, nurses, occupational therapists, and speech therapists.
Meet facility productivity expectations while maintaining high-quality patient care.
