Job Description:
Resolves patient issues autonomously and serves as the point of contact for all patient escalations. Assists with screenings per physician orders and accurately enters all information at the time of service or results. Plans proactively for each patient’s medical needs and follows through to a solution. Resolves problems, completes audits, identifies trends, determines system improvements, and implements changes. Provides guidance, motivation, and encouragement to guides to achieve goals. Coordinates patient care tasks and ensures adherence to care plans. Partners with and attends stakeholder meetings as necessary. Manages EHR and lab tasks, including entering labs, sending requisition forms, coordinating referrals, and disseminating order information and results to physicians and providers. Resolves order clarification issues, edits pharmacy information, and troubleshoots patient issues. Ensures the loop is closed with physicians and providers after referrals, including collecting patient notes from providers. Schedules follow-up appointments with patients, closes the loop with providers, and makes outbound calls to close care gaps in EHR. Performs clinical intake according to policy and procedures. Performs other duties as assigned.
