List of Job Duties:
-Complete Admission packets
-Assist in Care Plan Meetings
-Provide counseling and support to residents and families.
-Advocate for Resident Rights.
-Facilitate opportunities and concerns between all parties.
-Admission and Discharge Planning
-Collaborate with residents, families, and community resources for extended care.
-Update Care plans and MDS on Quarterly and Annual basis
-Address complaints and greivances
-Communicate effectively with all parties.
-Be POSITIVE
-Be PREPARE
-Be ORGANIZED
