Job Summary:
Directs nursing care for the patients and supervises the day-to-day nursing activities performed by assigned staff. Such supervision should be in accordance with federal, state, and local and regulations governing the nursing center. Also, as directed by the Administrator, the Medical Director, RN Charge Nurse and/or the Director of Health Services, to ensure the appropriate care for patients is provided.
Hours: 7:00 p.m.- 7:00 a.m. or 7:00 a.m. - 7:00 p.m.

Job Duties:
· Provides care ensuring patient/resident safety
· Completes documentation procedures on patients (appropriate use of forms, timelines, and Medicare documentation etc.)
· Assists in coordinate the care planning/MDS process and committee meetings as necessary.
· Assists in follow-up on consultant recommendations i.e..,pharmacy, dietary, etc.
· Assists in monitoring physician services (documentation and visits. etc) in accordance with current regulations.
· Knowledge of procedures and ability to determine Advance Directive status for patients.
· Communicates well with patients/residents/clients and family members providing warm and friendly
· Greeting and an approachable attitude to families, visitors, patients/residents/clients and responds to expressed concerns while displaying a helpful, caring demeanor. Answers questions when appropriate in a professional manner.
· Participates in center/agency surveys and any subsequently required reports.
· Admits, discharges and transfers patients as requested/necessary.
· Understanding of basic healthcare procedures, including medication administration.
· Collaborative skills to work effectively with the healthcare team.
· Strong communication, time management, organizational, and problem-solving abilities.
Requirements:
Possess a valid LPN nursing license in the State of Georgia.
Maintain current CPR certification.

Extensive knowledge of basic health care procedures
Knowledge of measuring and administering medication.
Physical Demands:
· Ability to lift, move, and reposition residents as needed.
· Ability to stand, walk, and bend for extended periods of time.
· Ability to respond quickly to medical emergencies.
Work Conditions:
· Work is typically performed in a skilled nursing facility setting, with potential exposure to bodily fluids, chemicals, and other hazardous materials.
· Full-time, part-time, or PRN shifts may be available, including weekends and holidays as needed.
Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
