Job Title: Integrated Care Nurse Practitioner
Location: Cedar Rapids, Iowa (must live within 20 miles)
Salary: $120,000 - $145,000
Job Overview:
The Advanced Practice Provider delivers personalized, value-based medical care to patients living with chronic kidney disease and end-stage renal disease. This is a field-based role providing in-home and virtual comprehensive assessments, care coordination, and preventative management. The APP works closely with field nurses, community health workers, and physicians to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.
This is not an office-based position. The provider must reside within 20 miles of Cedar Rapids, Iowa and be comfortable traveling within their assigned territory.
