OPEN POSITION:

  • Nurse Practitioner or Physician Assistant - Geriatrics / Palliative Care SCHEDULE: - Full-Time - 40 Hours per Week - Monday to Friday - No On-Call - No Weekends - No Evenings - No Holidays - Some Flexibility Offered COMPENSATION: - $135,000 to $150,000+ Annual Income - $60 to $65+ per Hour, negotiable based on experience - Quarterly Quality Bonuses - Daily Productivity Bonuses - Mileage Reimbursement - Malpractice Insurance - 34 Days Paid Time Off (Vacation / CME / Holidays) - Health / Dental / Vision / Disability / Life Insurance - $2,000 Annual CME Allowance - $500 Annual Professional Allowance (Memberships / Dues) - License / DEA Renewal Fees Paid - 403b w/ Employer Match - Company Cell Phone - Many More Perks & Benefits LOCATION: Greensboro, North Carolina COMPANY PROFILE: This compassionate non-profit is dedicated to delivering convenient, high-quality healthcare to patients in a variety of settings. They currently boast a team of 12 physicians, nurse practitioners, and physician assistants who, along with nursing, clinical, and administrative team members, ensure that patients receive the best medical care possible while working diligently to remove access barriers.

POSITION DESCRIPTION:

  • The Nurse Practitioner / Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting private homes, assisted and independent living facilities, and other group settings, The duties of the role include, but are not limited to: - conduct comprehensive physical examinations - diagnose and treat illnesses - manage chronic conditions - engage in advanced care planning discussions - coordinate care with other healthcare providers - provide palliative care consultations Providers see an average of six (6) patients per day. Based on patient need, more visits may be scheduled and are reimbursed per visit in addition to the hourly rate.

The new provider will see medically complex, frail, geriatric adults who may have:

  • - high clinical complexity - multi-morbidity - functional decline - mobility limitations - polypharmacy - cognitive impairment - caregiver burden - increased risk for avoidable utilization - evolving goals of care This requires providers to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education as needed.
  • The practice uses the AthenaCare EHR.

REQUIREMENTS:
