Job description

SUMMARY

Integrated Primary Care Clinical Pharmacists will practice centrally within the Baptist Health Population Health Services Organization (PHSO) or embedded within a multidisciplinary clinical team to support primary care clinics. The primary responsibility of this pharmacist is to assure the provision of safe, efficacious, cost-effective, and high-quality medication therapy disease management to patients in the ambulatory setting. The pharmacist in this role will interact with patients and providers remotely and face-to-face as needed. The pharmacist will consult with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals. The pharmacist assesses each of the patient’s medications to determine that each is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, cost effective and able to be taken by the patient as intended. The pharmacist will titrate, manage, and monitor the patient’s medication therapy to optimize clinical outcomes and will identify and address gaps in care. Additional responsibilities include patient and provider education, medication management of chronic disease, comprehensive medication reviews, transitions of care, motivational interviewing, precepting students and residents, and assisting in the development of network resources and initiatives. Pharmacists must have a patient-centered mindset, excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.

RESPONSIBILITIES

  • Punctual and regular attendance
  • Assumes responsibility for the delivery of high-quality medication disease management services for the specific patient care areas in which the pharmacist is assigned.
  • Demonstrates proficiency in medication and disease management, as well as a thorough understanding of the medications, clinical conditions, and current treatment guidelines for the conditions in their assigned clinical areas.
  • Gathers appropriate clinical and laboratory data necessary to make appropriate decisions on the management of the patient’s medication therapy and related diseases and conditions.
  • Assesses patient’s medication therapy regimen for appropriateness, effectiveness, tolerability, safety, and potential for adverse events.
  • Make clinically appropriate and cost-effective changes to the medication regimen to maximize effectiveness and patient adherence, while working to minimize potential for both acute and chronic complications
  • Provides in-services, in-depth drug information, and other educational and informational materials to members of the medical, nursing and other healthcare professionals.
  • Provides patients with appropriate educational materials and information in order to help them understand and actively participate in their care.
  • Appropriately documents each patient encounter in the electronic health record and conveys that information to the referring physician and next provider of care as appropriate.
  • Attends, participates, and upon request, presents at department quality meetings.
  • Advises, assists, and supervises the work of PHSO pharmacy technicians; assumes final responsibility for accuracy of their work.
  • Works with clinic medical and nursing staff, support staff as well as PHSO and AHG/BHS administration where necessary to resolve issues, incorporate clinic services seamlessly into site workflow to continually demonstrate the value of the ambulatory clinical pharmacy program.
  • Work is typically performed in a clinical environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures. MINIMUM REQUIREMENTS

  • Graduate of an ACPE accredited College of Pharmacy with a Doctorate of Pharmacy degree
  • Active license to practice Pharmacy from the Arkansas State Board of Pharmacy
  • 3 years’ experience as a full-time pharmacist or completion of ASHP-accredited PGY1 residency program
  • Strong verbal and written communication skills
  • Eligible to precept APPE students and PGY1 pharmacy residents as defined by latest ASHP standards

PREFERRED QUALIFICATIONS

  • Completion of an ASHP-accredited PGY1 residency
  • Completion of an ASHP-accredited PGY2 in ambulatory care
  • Board Certification (BCPS or BCACP) • Quality improvement experience
