About this role:

As an Advanced Practice Provider in Hospice Medicine,you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will manage hospice patients across Utah County and Heber.

• This position includes a up to$7,500 in relocation assistancewhen applicable

• You willworka hybrid role with offices based in Provo, UT and Heber, UT

• Your hours will beMonday through Friday, 8am-5pm

• You will be on call 1 week every 4-6 weeks from 5pm – 8am

What you’ll bring:

• APRN/ NP degreefrom an accredited program

• Minimum 1 year of experience in end of life care, required

  New grads willnotbe considered

• Active, unobstructed Utah medical license, or the ability to obtain one

• Ability to successfully complete Intermountain Health’s credentialing process

Life in Provo, UT

Provo is the heart of Utah County with a population exceeding 113K. It is one of the best places to live in the US with abundant seasonal recreation, great schools and a reasonable cost of living. It is a bustling community that provides the advantages of a small-town lifestyle along with the amenities offered in more cosmopolitan areas. Exciting outdoor adventures include hiking, backpacking, sightseeing, and skiing. Utah Lake, just west of Provo, provides excellent water sports, including fishing. As the home of Brigham Young University, and Utah Valley University 5 miles away, Provo offers outstanding college sporting events and several cultural entertainments. It is a short 40-minute drive from Salt Lake City, which hosts a variety of professional sports and cultural events.

Life in Heber, UT

The Heber Valley is nestled in Utah's Wasatch Mountains only 45-minute drive from Salt Lake City and adjacent to Park City. It was a venue for both cross-country skiing and biathlon during the 2002 Olympic Winter Games. It continues to offer convenient access toPark City Mountain Resort, Deer Valley Resort, and the Canyons. It also features five championship golf courses and blue-ribbon fly-fishing. Year-round activities include snowmobiling, downhill and cross-country skiing, horseback riding, mountain biking, camping, boating, hot air ballooning, water sports, scuba diving, and more. In addition, unique area attractions include the historic, steam-driven Heber Valley Railroad, beautiful Cascade Springs and other scenic drives, three reservoirs, four ski resorts and three national forests/state parks within 45-50 minute drive. Heber Valley Hospital is a 19-bed facility, housing three operating rooms, one procedure room, nine surgical recovery rooms, seven ER treatment rooms, and 170 employees. It offers an Urgent Care clinic, full service emergency department, 24/7 lab,dexa-scanner, digital mammography, and physical therapy.
