Licensed Practical Nurses (LPN's)
Concord Center for Nursing and Rehabilitation is actively hiring LPN Team Members for our Skilled Nursing Facility in Brooklyn, NY.
Now Offering a $7,500 Sign-On Bonus, which is paid out in installments over the first year of employment.
Base rate is $33.54 per hour with an additional 10% shift differential for evenings and nights ($36.89.)

DUTIES:
Collecting required information from new Residents to be admitted
Recording health details of Residents; including vitals & temperature
Administering medications, wound care, clinical documentation, resident assessments and care coordination.
Provide clinical direction and support to Certified Nursing Assistants while coordinating resident care during assigned shift.
Monitors Residents’ food and liquid intake and output
REQUIREMENTS:
Must be able to work as a team member
Valid NY State LPN license
IND123
Must be in good standing with State Registry
