Job description
PAY IS PER VISIT NOT PER HOURHuman Touch Home Care is hiring Licensed Practical Vocational Nurses in the Colorado area. The Licensed Practical Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities including following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.Benefits: ONLY FOR FULL TIME EMPLOYEESCompetitive salary commensurate with experience.Opportunities for professional development and career advancement.Positive and supportive work environmentContribution to improving healthcare access and quality in the communityComprehensive benefits package including:Health insuranceVisionDentalPaid Time OffSick LeaveRetirement plansResponsibilities:Provides direct patient care as defined in the state Nurse Practice Act.Implements plan of care initiated by the registered nurse.Provides accurate and timely documentation consistent with the plan of care.Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.Participates in personal and professional growth and development.Performs other duties as assigned by the registered nurse.The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job related tsks other than those stated in this description.QualificationsGraduate of an accredited practical nurse or vocational nursing program.Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.Possess and maintain valid CPR certificationplies with accepted professional standards and practice.Demonstrates good verbal and written communication and organizational skills.Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.Experience:LPN: 2 years (Required)Home health: 1 year (Preferred)License/Certification:Driver's License (Required)LPN (Required)Job Types: FulltimePay: $30.00-45.00 per visit (typical visit 1 hour)Medical specialties: Home HealthWork setting: In-personAbility to commute: Colorado Springs, CO: Reliably commute (Required)Note - This job description is not intended to be all-inclusive.
