Salary: $65K-$70K; Depending on experience
Qualifications:
Prior experience as a Director of Case Management in an assisted living setting strongly preferred
Must have healthcare experience as a Case Manager
Must have clinical knowledge
Must have prior experience in geriatric population
Strong written and verbal communication skills
Responsibilities:
Oversee and manage all aspects of case management for the facility, ensuring compliance with state and federal regulations.
Develop, implement, and monitor individualized care plans for residents in collaboration with medical staff, families, and caregivers.
Act as a liaison between residents, families, healthcare providers, and community resources to facilitate optimal care and services.
Train, lead, and support a team of case managers and related staff to maintain high standards of care.
Monitor residents’ progress and make necessary adjustments to care plans to ensure their needs are consistently met.
Maintain accurate and up-to-date documentation and reports related to case management activities.
Address and resolve any concerns or issues related to resident care in a professional manner
