Join a mission-driven team as a Primary Care Physician providing comprehensive, in-home medical care to medically complex, homebound patients. This role includes a pathway to a supervisory Medical Director position after a successful probationary period, offering strong leadership, autonomy, and meaningful patient impact.
Key Responsibilities:
- 🏠 Provide primary medical care to patients in home-based settings
- 📋 Conduct comprehensive assessments: history, physical exams, medication review
- 💊 Diagnose and manage acute and chronic conditions; prescribe medications
- 🤝 Coordinate care with interdisciplinary teams: APPs, nurses, and community health workers
- 🗣️ Educate patients and families on disease management, medications, and lifestyle modifications
- 📝 Maintain accurate and timely documentation for all visits
- ⚡ Address urgent medical needs during home visits and arrange referrals as needed
- 🏥 Support additional services: PT, labs, home health nursing
- 🌟 Mentor, guide, and supervise interdisciplinary team members
- 📊 Assist with administrative oversight and team leadership

Requirements
- 🎓 MD or DO from an accredited U.S. medical school
- Completion of Internal Medicine or Family Medicine residency
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active New York State medical license
- DEA registration
- Minimum 5 years post-training clinical experience
- Experience providing primary and diagnostic care
Preferred Skills:
- Geriatric medicine or palliative care experience
- Experience working with homebound or complex patient populations
