Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 year of Home Health PT experience License: CA PT License, CA driver’s license, and auto insurance required Certifications: BLS preferred Must-Have: - Required: CA PT License - At least 1 year of experience as a Home Health PT - CA driver's license & auto insurance Description: The position is for a Physical Therapist specializing in Home Health. The therapist will handle approximately 4-5 patients per day. A minimum of 1 year of experience in home health physical therapy is required. Candidates must possess a valid California Physical Therapy license. A valid California driver’s license and auto insurance are mandatory. Basic Life Support (BLS) certification is preferred. The therapist will deliver care to adult and geriatric patients. Daily patient visits will be around 5 on average. The role does not require any weekend or night shifts. There is no float or call requirement associated with the position. Experience with specific charting systems is preferred. Clinical skills in various areas, including mobility training and pain assessment, are essential. The therapist will develop individualized care plans and assess patient progress. Adaptability to different patient needs and environments is crucial. Strong communication skills for patient family education and coordination of care are necessary. The position emphasizes team collaboration and interdisciplinary support. Candidates should demonstrate proficiency in adaptive devices and safety protocols. It is important to have a clear awareness of patient safety measures and injury prevention strategies.
