Integrated Behavioral Health Clinician Embedded in Family Medicine Practice

• Candidate Qualifications:
• Licensed therapist (LCSW, LCPC, LMFT) skilled in the delivery of evidence-based interventions. License to practice in Illinois required.
• REMOTE position, but will join on site meetings occasionally when needed in Springfield, IL
• Ability to see ages 12+
• Experience in a fast-paced, multidisciplinary care setting required, integrated care or primary care settings preferred
• Ability to develop and implement individualized treatment plans that address behavioral, physical health, and social needs
• Strong interpersonal and communication skills
• Ability to work effectively in a fast-paced, team-based environment
• Proficiency working within electronic health record systems and efficiency in documenting in a clear, concise, timely manner
• A passion for expanding access to quality care through innovative, cutting-edge treatment approaches
