Requirements:

  • Board Eligible (within 5 years of residency) or Board Certified in Hematology and Oncology - [REQUIRED] *no exceptions
  • Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)- [REQUIRED]
  • Clean malpractice/license history - [HIGHLY PREFERRED]
  • Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at the presentation
  • DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
  • Permanent Placement Fee $25K (note in "Additional Submission Details" field) - [REQUIRED]
  • Visa Support: Does provider require J1 or H1B support? (note in "Additional Submission Details" field)- [REQUIRED]
