We are seeking a self-motivated, relationship-driven Hospice Clinical Liaison to join our Sacramento and surrounding counties team. This is an exciting opportunity for a dynamic professional who thrives on building meaningful partnerships and driving growth in a collaborative, mission-driven environment.
Position Summary
The Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners.
The ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process.
